A very well known and competent guide in Cordillera Blanca is Richard Hidalgo. I can put you in contact with him.

I've been very happy with Skyline Adventures (http://www.skyline-adventures.com/); excellent service and good prices (Peruvian prices, not US or GB prices). If you just want a guide, I would recommend Koky Casteneda - I can get you contact information if you are interested.
