I recently changed the registration process again to make it harder for bots to automatically register and when I did that I eliminated the manual approval process for accounts. So these guys are spending the human time to get past the CAPTCHA in order to use a bot to automatically spam. Annoying. But I think it's more annoying for new users to have to wait sometimes days to get their account approved. So we'll deal with the spam when it happens.

I know some websites do this, but I don't know how it is done.

All the mass spammers are from Korea and China it seems. I don't think we have ever had valid forum posts using those characters.

Or require a Captcha to complete a post that contains suspicious characters. I know we sometimes have legit posts about Chinese and Korean mountains, and I'm pretty sure I've used a Chinese character or two myself.
