Green trails makes one with all the climbing routes on it, as well as the entire timberline trail.

It's called Green Trails map 462S

And is located here.

I have also seen them at REI, OMC, Mountain Shop, Next Adventure Locally (Portland)

I have that one. I've not yet actually hiked or climbed Mt Hood but the map seems pretty good.
